SHORE YOUTH LACROSSE 2015 SPRING REGISTRATION

OPEN TO BOYS /GIRLS 1^ST THRU 8^TH GRADE

OPEN TO CHILDREN RESIDING IN OCEANPORT, MONMOUTH BEACH, WLB, AND SEA BRIGHT

BOYS/GIRLS  1^ST AND 2^ND GRADE:  Full Equipment. Instructional, focusing on fundamentals. Practice days to be announced.

BOYS 3^RD AND 4^TH GRADE, 5^TH AND 6^TH GRADE, AND 7^TH AND 8^TH GRADE TEAMS: Full Equipment.  Practice and game instruction provided by United Lacrosse! Although subject to change, practices will be on Wednesdays and Fridays from 4pm to 530pm. Games on Saturdays and/or Sundays

GIRLS 3^RD AND 4^TH GRADE, 5^TH AND 6^TH GRADE, AND 7^TH AND 8^TH GRADE TEAMS: Parent coached, two practces per week, with one practice instruction provided by United Lacrosse!

**Please note that equipment (except for goalies) is not provided. Required equipment for boys includes stick (no mini sticks), helmet, mouth guard, gloves, chest/shoulder pads, elbow pads, athletic supporter with cup. Required equipment for girls includes stick, mouth guard, and goggles.

EACH CHILD IS REQUIRED TO BECOME ACTIVE US LACROSSE MEMBERS WHICH PROVIDES INSURANCE! COST IS $25 FOR ONE YEAR MEMBERSHIP .  REGISTER AND PAY ONLINE AT WWW.USLACROSSE.ORG   CHILDS VALID US LACROSSE MEMBERSHIP NUMBER must be entered online when you register your child (must not expire before 6/14/2015). Please designate “Shore Youth Lacrosse” as your child’s team when registering with US Lacrosse!

REGISTRATION FEES:

3^RD THRU 8^TH GRADE BOYS:  $275 .  3^RD THRU 8^TH GRADE GIRLS: $150

1st AND 2^ND GRADE BOYS/GIRLS:  $75      NEW: 3^rd and 4^th GRADER BOYS PRACTICE TEAM ONLY: $75

STRICT DEADLINE: January 14th, 2015. PLEASE SIGN UP EARLY!  REGISTRATION CLOSES AFTER THE JANUARY  14^TH DEADLINE.   AFTER JANUARY 14^TH, PARTICIPATION IS NOT GUARANTEED!!
